The methodology flaws in Hinman's acupuncture clinical trial, Part III: Sample size calculation.

چکیده

In the October 2014 publication of JAMA, Dr. Hinman and colleagues published the study “Acupuncture for Chronic Knee Pain: A Randomized Clinical Trial,” in which the authors concluded that “in patients older than 50 years with moderate or severe chronic knee pain, neither laser nor needle acupuncture conferred benefit over sham for pain or function. Our findings do not support acupuncture for these patients”. We strongly disagree with such a conclusion. As pointed out in my former articles, serious flaws exist in the trial design, statistics, interpretation of the results, as well as the use of Zelen study design itself, as reported by Hinman et al. In Part II of the critique I discussed problems in sample size calculation of the Hinman study due to the variance introduced by multiple groups, multiple therapists, as well as the characteristics of intervention therapies. In this article, I discuss these aspects in more detail.

Acupuncture for Chronic Knee Pain: A Critical Appraisal of an Australian Randomized Controlled Trial

Background: Chronic knee pain caused by osteoarthritis (OA) is common in the general population. For knee OA with chronic moderate-to-severe pain, acupuncture treatment is recommended. Previous randomized controlled trials (RCTs) and meta-analyses have indicated that acupuncture can decrease pain and improve function in patients with knee OA. Interestingly, a recent Australian RCT by Hinman et ...

Sample size calculation in clinical trials: part 13 of a series on evaluation of scientific publications.

BACKGROUND In this article, we discuss the purpose of sample size calculation in clinical trials, the need for it, and the methods by which it is accomplished. Study samples that are either too small or too large are unacceptable, for clinical, methodological, and ethical reasons. The physicians participating in clinical trials should be directly involved in sample size planning, because their ...

A mixed Bayesian/Frequentist approach in sample size determination problem for clinical trials

In this paper we introduce a stochastic optimization method based ona mixed Bayesian/frequentist approach to a sample size determinationproblem in a clinical trial. The data are assumed to come from a nor-mal distribution for which both the mean and the variance are unknown. In contrast to the usual Bayesian decision theoretic methodology, whichassumes a single decision maker, our method recogni...
